Quality System Liaison

Description

Responsible for QA and PI activities, JCAHO readiness for Home Care & Hospice. Assures the information system meets the clinical and business requirements of the Home Care and Hospice departments. Serves as a communication intermediary between the Home Care & Hospice and the Information Services Department. Provides training, consultation, and coordination of all projects and upgrades. Provides ongoing maintenance of system software and hardware. Provides quick response for resolution of any issues, to ensure clinicians are able to perform patient care activities timely.

Qualifications

  • Associate Degree in Nursing required
  • Bachelor's Degree in Nursing preferred
  • Two (2) year's home-care nursing and/or three (3) years of continuing education/staff development education required
  • Experience equivalent to computer proficiency required
  • Informatics experience with performance improvement activities required
  • Previous experience with performance improvement activities required
  • Must possess knowledge of Medicare conditions of participation, JCAHO standards, OASIS and PPS
  • Overall baseline understanding of home care/hospice operations, reimbursement and clinical process preferred
  • Demonstrating knowledge of Allscripts (Misys) Home Care software preferred
  • Must possess excellent interpersonal, verbal, and written communication skills.
  • Must have excellent organizational and prioritization skills
  • May be required to travel to multiple local sites
  • Must have ability to multi-task and work under pressure while maintaining a pleasant and approachable demeanor.
  • Must have ability to work independently and as an effective team member
  • Must have ability to be flexible in job duties and work hours
  • Must be physically and mentally able to perform all essential job functions

Distinctive working conditions: The environment of home care/hospice may be unsanitary and/or the neighborhood unsafe. Travel may be required in inclement weather conditions, causing a safety hazard.

Must have own vehicle and liability care insurance of an amount not less than $100,000 individual and $300,000 occurrence.

Comments: 40 hours per week
